Eligibility
To be eligible to receive DVA Community Nursing services you must hold either Gold or White card. You must also undergo an assessment to determine whether you need Clinical or Personal care
Gold Card holders: The DVA will fund Community Nursing Services that meet your assessed personal care and/or clinical requirements.
White card holders: The DVA will only fund Community Nursing Services if they are required due to an accepted disability, this includes war or service caused injury or disease.
If you are unsure of your eligibility for the program, Dial Care Services can contact DVA on your behalf for confirmation. If you need extra support, we can also advocate for you.
Costs
If you are assessed as eligible for DVA Community Nursing Program, there is no cost for you.
Referrals
- Local Medical Officer (LMO) or other General Practitioner (GP), 
- Nurse Practitioner specialising in a community nursing field, 
- treating doctor in hospital, 
- hospital discharge planner, or 
- Veterans’ Home Care (VHC) Assessment Agency
